unique_filename doesn't support sub-folders

The 'upload' function supports passing in a sub-folder, but when the filename is checked for uniqueness, unique_filename() doesn't check the correct sub-folder; it checks in the root of the uploads folder. So filenames in sub-folders are getting renamed so that they don't collide with filenames in /uploads.
